Root canal treatment, also called endodontics, is the procedure that removes the pulp tissue (nerve and blood vessels) from inside the tooth when it is inflamed or infected. After the canal is cleaned and disinfected, it is filled with an inert material and the tooth is restored — and it can last decades or even a lifetime.
No — root canal treatment does not hurt. The pain felt before coming to the clinic is caused by the inflammation or infection inside the tooth. During treatment, we apply an effective local anaesthetic that completely removes the sensation. Most patients are surprised by how comfortable the procedure is. Afterwards there may be some residual sensitivity for 24 to 48 hours, easily managed with a common painkiller.
At Porto Smile we complete the vast majority of root canal cases in a single session. In cases of active infection, a heavily calcified canal or the retreatment of a previous canal, more than one visit may be needed. The number of sessions is determined at the diagnosis appointment, based on the clinical and radiographic assessment.
Root canal treatment is indicated when the tooth's nerve is inflamed (pulpitis) or dead (pulp necrosis). The most common signs are: spontaneous or persistent pain, pain to heat or cold that does not go away, pain when biting down, swelling of the gum next to the tooth or a change in the tooth's colour. A deep cavity that has reached the pulp is another common indication.
Treating the canal is almost always the best option. A natural tooth is superior to any replacement — implant, bridge or denture. Losing a tooth triggers progressive bone loss and can compromise the neighbouring teeth. Root canal treatment preserves the tooth, its chewing function and the bone structure. Only when the tooth is beyond saving does extraction become unavoidable.
A properly treated tooth can last decades or even a lifetime. Its longevity depends on the quality of the treatment, the restoration carried out after the canal (a weakened tooth may need a crown) and the patient's oral hygiene. With appropriate regular maintenance, the prognosis is excellent in the vast majority of cases.
Removing the pulp makes the tooth slightly drier and more brittle — so in many cases, especially in the back teeth subject to high chewing forces, a crown is recommended after the canal. It protects the tooth and ensures longevity. The dentist assesses case by case which restoration is most suitable after treatment.
